长期以来,诊断明确、病情稳定的慢性病、康复期患者难以从三级医院及时转至基层医疗卫生机构,致使急危重症和疑难杂症患者享受不到三级医院的优质医疗资源,而基层医疗卫生机构又门可罗雀,医疗资源综合利用效率和效能低下。为解决上述问题,以镇江市第一人民医院建立由社区直接挂号三级医院的双向转诊系统为例,介绍通过社区医生直接在社区操作挂号三级医院,实现社区和三级医院之间病人信息的共享,方便病人就医,简化由社区转诊到三级医院的流程,实现了上下级医疗机构间的无缝对接,提高了医院医疗服务水平。
For a long time, the diagnosis is clear and stable chronic diseases. Patients in rehabilitation period are difficult to transfer from tertiary hospitals to primary health care institutions in time. As a result, patients with acute and difficult incurable diseases can not enjoy the high-quality medical resources of tertiary hospitals, while primary medical care Health institutions are beyond recognition, and the comprehensive utilization of medical resources is inefficient and ineffective. In order to solve the above problems, taking the two-way referral system established by the first directly affiliated hospital in Zhenjiang City as an example, this paper introduces that the community doctor can directly operate the registered third-grade hospital in the community to realize the relationship between the patients in the community and tertiary hospitals The sharing of information facilitates the patients to seek medical treatment, streamlines the referral from the community to tertiary hospitals, realizes the seamless connection between higher and lower level medical institutions and improves the level of medical services in hospitals.
