1型糖尿病患者23例,随机分为二甲双胍组(11例)和利拉鲁肽组(12例),二甲双胍组在胰岛素强化治疗基础上加用二甲双胍,起始剂量0.25g/次,3次/d,耐受后增至0.5 g/次,3次/d;利拉鲁肽组在胰岛素强化治疗的基础上加用利拉鲁肽,剂量0.6mg,皮下注射,1次/d。治疗12周后,观察两组患者空腹血糖(FPG)、餐后血糖(2h PG)、糖化血红蛋白(Hb A1c)、体质量、TG、TC、SBP和DBP及血糖的稳定性。结果治疗12周后,两组血糖、Hb A1c均显著降低(P<0.01),体质量、BMI及血浆TG、TC无明显变化(P>0.05);利拉鲁肽组降低FPG、Hb A1c的效果与二甲双胍组相当(P均>0.05),但降低2h PG及降低SBP的效果优于二甲双胍组(P均<0.05)。利拉鲁肽组患者血糖水平和达标率明显优于二甲双胍组(P<0.01)。结论利拉鲁肽可有效控制血糖,降低SBP,不增加低血糖风险,增加血糖的稳定性,适用于1型糖尿病患者。
Twenty-three patients with type 1 diabetes were randomly divided into metformin group (n = 11) and liraglutide group (n = 12). Metformin group was supplemented with metformin on the basis of intensive insulin therapy. The initial dose was 0.25 g / d, tolerated to 0.5 g / time, 3 times / d; liraglutide group plus liraglutide on the basis of intensive insulin treatment, a dose of 0.6mg, subcutaneous injection, 1 time / d. After 12 weeks of treatment, the stability of fasting blood glucose (FPG), postprandial blood glucose (2h PG), Hb A1c, body weight, TG, TC, SBP, DBP and blood glucose were observed. Results After 12 weeks of treatment, the levels of Hb A1c and Hb A1c in both groups were significantly decreased (P <0.01), and there was no significant difference in body weight, BMI, plasma TG and TC between the two groups (P> 0.05) The effect was similar to that of metformin group (all P> 0.05), but the effect of lowering PG and decreasing SBP was better than that of metformin group (all P <0.05). Patients in liraglutide group had better blood glucose level and compliance rate than metformin group (P <0.01). Conclusion Liraglutide can effectively control blood glucose, reduce SBP, do not increase the risk of hypoglycemia, increase the stability of blood glucose, and is suitable for type 1 diabetes.
