目的研究和评价慢性阻塞性肺病(COPD)急性发作期患者营养状态对免疫功能的影响。方法通过人体测量和内脏蛋白测定对53例COPD患者进行营养学评价与分类。营养正常者16例(30%)、中度营养不良者23例(43.4%)、重度营养不良者14例(26.4%),并测定三组患者体液免疫功能和细胞免疫功能。结果除IgG、IgA、IgE无明显变化外(P>0.05),三组患者的T细胞亚群、淋巴细胞总数、OT试验等均随着营养状况的恶化而显著异常或下降(P<0.05~P<0.001)。结论提示当COPD患者营养状况恶化时,其细胞免疫功能将受到明显损害。
Objective To study and evaluate the effect of nutritional status on immune function in patients with acute exacerbation of chronic obstructive pulmonary disease (COPD). Methods Nutritional evaluation and classification of 53 patients with COPD were performed by human body measurements and visceral protein assays. 16 cases (30%) with normal nutrition, 23 cases (43.4%) with moderate malnutrition, and 14 cases (26.4%) with severe malnutrition. The humoral immunity and cellular immunity were measured in three groups. Results There was no significant change except IgG, IgA, IgE (P> 0.05). The T lymphocyte subsets, total lymphocytes and OT in three groups were significantly abnormal or decreased with the deterioration of nutritional status (P < 0.05 ~ P <0.001). Conclusion suggests that when the nutritional status of COPD patients deteriorate, their cellular immune function will be significantly impaired.
