目的探讨盆腔器官脱垂(pelvic organ prolapse,POP)患者骶韧带成纤维细胞粘弹性变化。方法选取2009年12月至2010年6月河北医科大学第二医院妇产科POP患者11例为观察组,同期非POP患者13例为对照组,取骶韧带进行成纤维细胞的原代培养。光镜观察与免疫细胞化学染色法进行成纤维细胞鉴定,取1代的成纤维细胞(实验组观察细胞数为18,对照组观察细胞数为22),采用微管吸吮技术表征骶韧带成纤维细胞的粘弹性特性。结果培养的细胞呈梭形,表达波形蛋白(vimentin),POP患者骶韧带成纤维细胞粘弹特性明显低于正常对照组,病例组骶韧带成纤维细胞平衡模量[E∞=(124.62±20.22)Pa]、瞬间模量[E0=(429.25±105.89)Pa]、黏性系数[μ=(718.53±159.08)Pa.s]均显著低于对照组[E∞=(170.17±24.65)Pa]、[E0=(495.26±69.39)Pa、μ=(845.35±155.34)Pa.s],P<0.05。结论 POP患者成纤维细胞骨架结构和机能状态的改变可能是POP发生的关键环节。
Objective To investigate the viscoelasticity of sacral ligament fibroblasts in patients with pelvic organ prolapse (POP). Methods Eleven patients with POP in obstetrics and gynecology department of the Second Hospital of Hebei Medical University from December 2009 to June 2010 were selected as the observation group and 13 non-POP patients in the same period as the control group. The primary culture of fibroblasts was performed with sacral ligaments. Fibroblasts were identified by light microscopy and immunocytochemical staining. Fibroblasts of 1st generation (18 cells observed in the experimental group and 22 cells observed in the control group) were characterized by microtubule sucking technique to characterize the fibrosis of the sacral ligament Viscoelastic properties of cells. Results The cultured cells were fusiform in shape and vimentin was expressed. The viscoelastic properties of sacral ligament fibroblasts in POP patients were significantly lower than those in normal controls (E∞ = (124.62 ± 20.22 ) Pa], the instantaneous modulus [E0 = (429.25 ± 105.89) Pa] and the viscosity coefficient [μ = (718.53 ± 159.08) Pa.s] were significantly lower than those in the control group [E∞ = (170.17 ± 24.65) Pa] , [E0 = (495.26 ± 69.39) Pa, μ = (845.35 ± 155.34) Pa.s], P <0.05. Conclusion The changes of skeletal structure and functional status of fibroblasts in patients with POP may be a key link in the pathogenesis of POP.
