目的:探讨原发性肝癌(primary hepatic carcinoma,PHC)的血流灌注特点。方法:对94例PHC及29例无肝脏疾病患者(对照组)进行肝脏CT灌注扫描,采用斜率法计算其肝动脉灌注量(hepatic arterial perfusion,HAP)、门静脉灌注量(hepatic portal perfusion,HPP)、总肝灌注量(total liver perfusion,TLP)、肝动脉灌注指数(hepatic arterial perfusion index,HAPI)和门静脉灌注指数(hepatic portal perfuiosn index,HPPI),对比分析PHC和对照组血流灌注参数差异。结果:PHC组织HAP和HAPI显著高于对照组(p<0.0001),HPP、TLP和HPPI显著低于对照组(p<0.0001)。结论:CT灌注成像可以精确测定PHC血流灌注参数;PHC表现为高动脉灌注和低门静脉灌注。
Objective: To investigate the perfusion characteristics of primary hepatic carcinoma (PHC). Methods: CT perfusion scans were performed in 94 patients with PHC and 29 patients without liver disease (control group). Hepatic arterial perfusion (HAP) and portal hepatic perfusion (HPP) were calculated by slope method. The total hepatic perfusion (TLP), hepatic arterial perfusion index (HAPI) and portal perfusion index (HPPI) were compared to analyze the difference of blood perfusion parameters between PHC and control groups. RESULTS: HAP and HAPI in PHC tissues were significantly higher than those in controls (p<0.0001), and HPP, TLP, and HPPI were significantly lower than those in controls (p<0.0001). Conclusion: CT perfusion imaging can accurately determine PHC perfusion parameters; PHC showed high arterial perfusion and low portal vein perfusion.
