Ionized calcium, which is a physiologically significant component of serum calcium, is usually maintained in a very narrow range of concentrations by a well-controlled, stable mechanism. Some life-sustaining processes (eg, nerve conduction, muscle contraction, etc.) are regulated by this calcium concentration in serum. Above or below the tolerable limits will cause fatal changes in these life-sustaining activities. Therefore, the detection of serum calcium in patients with hypercalcemia or hypocalcemia is very important. The authors examined the affinity of serum proteins bound to calcium in patients with hyperparathyroidism, multiple myeloma, diabetes mellitus, osteoporosis and cirrhosis using gel filtration techniques such as Toffaletti et al.
