目的 :探讨食管癌切除术后减少脓胸发生的经验。 方法 :对术中污染较重者 ,用碘酒局部消毒 ,关胸前应用 0 .0 5 %的洗必太、0 .1%替硝唑氯化钠液冲洗胸腔 ,胸腔及纵隔内置入抗生素 ,根据情况置入纵隔引流管 ,术后选用有效抗生素。 结果 :连续手术切除食管癌 6 10例无脓胸发生 ,3例纵隔感染后痊愈。 结论 :充分术前准备 ,术中严格无菌操作。关胸前抗生素液反复冲洗胸腔 ,确保胸腔、纵隔引流通畅 ,选用有效抗生素及加强术后管理可防止脓胸的发生。
Objective: To explore the experience of reducing empyema after resection of esophageal cancer. Methods: Severe intraoperative pollution, partial disinfection with iodine, off the chest before the application of 0.5% chlorhexidine, 0.1% tinidazole sodium chloride flushing thoracic cavity, chest and mediastinum built antibiotics , According to the situation into the mediastinal drainage tube, effective use of antibiotics after surgery. Results: There was no empyema in 6 10 cases of esophageal cancer and 3 cases recovered after mediastinal infection. Conclusion: adequate preoperative preparation, intraoperative strict aseptic operation. Off the chest antibiotic solution repeatedly rinse the chest to ensure that the chest, mediastinal drainage patency, use of effective antibiotics and strengthen postoperative management can prevent the occurrence of empyema.
