目的:本文报道CO_2激光治疗耳鼻咽喉部乳头状瘤172例,并取得满意的疗效,它是一种简单和有效的方法。方法:172例鼻部乳头状瘤45例,耳部乳头状瘤39例,咽部乳头状瘤82例,喉部乳头状瘤6例。治疗:局部或全身麻醉后,以连续输出功率为4~20W的CO_2激光切割及气化肿瘤,切取获得标本可供病理切片。术后创面有一层凝固膜,约2~3周,创面痂膜自行脱落,周围上皮移行愈合,创面伤疤平整。结果:治愈率达100%。讨论:CO_2激光治疗耳鼻咽喉部乳头状瘤的优点:(1)生物组织含70%~90%的水分,激光气化肿瘤,对邻近组织基本不受影响,不妨碍伤口Ⅰ期愈合,(2)激光切割和气化肿瘤后,可最大限度保存原有器官的功能,不造成组织缺损,(3)激光能封闭和凝固肿瘤组织周围的血管,术时出血少,(4)激光是无菌切口,很少引起伤口感染,(5)手术简单,痛苦少。
OBJECTIVE: In this paper, 172 cases of otolaryngologic and nasal laryngeal papilloma were treated with CO2 laser and satisfactory curative effect was obtained. It is a simple and effective method. Methods: 172 cases of nasal papilloma, 45 cases of ear papilloma in 39 cases, pharyngeal papilloma in 82 cases, 6 cases of laryngeal papilloma. Treatment: After local or general anesthesia, with a continuous output power of 4 ~ 20W CO_2 laser cutting and gasification of the tumor, cut specimens obtained for pathological sections. The wound has a layer of coagulation film, about 2 to 3 weeks, the wound callus film off itself, the surrounding epithelial transition healing, wound scar formation. Results: The cure rate of 100%. Discussion: CO_2 laser treatment of otolaryngology and laryngeal papilloma advantages: (1) biological tissue containing 70% to 90% of water, laser gasification of the tumor adjacent to the basic unaffected, does not prevent the wound healing, (2 ) Laser cutting and gasification of the tumor, can maximize the preservation of the original organ function, does not cause tissue defects, (3) the laser can close and coagulate the blood vessels around the tumor tissue, less bleeding surgery, (4) the laser is a sterile incision , Rarely cause wound infection, (5) surgery is simple, less painful.
