目的 探讨上颌窦底提升术在早期上颌牙龈癌手术治疗中的应用价值。方法 选择 7例局限于上颌牙龈部的早期牙龈癌 (男 5例 ,女 2例 ) ,于上颌窦前壁开窗 ,行上颌窦底粘膜提升后 ,矩形扩大切除窦底上颌骨及肿瘤 ,腭瓣或颊脂垫瓣修复创面。结果 1例上颌窦底粘膜破损 ,其余 6例病人上颌窦底提升顺利。随访 1～ 2年无复发 ,近期效果满意。结论 该术式符合无瘤操作原则 ,具有切除病灶彻底 ,又尽量保瘤上颌骨功能的优点 ,尤其有利于修复重建 ,具有一定的临床推广价值。
Objective To investigate the value of maxillary sinus floor lifting in the surgical treatment of early maxillary gingival cancer. Methods Seven cases of early gingival carcinoma (5 males and 2 females) confined to the maxillary gingival part were selected. The anterior wall of the maxillary sinus was opened. After maxillary sinus mucosa was elevated, the maxillary sinus was expanded and resected. The palate The flap or buccal fat pad flap repairs the wound. Results One case of maxillary sinus mucosa was damaged, and the other six patients with maxillary sinus floor to enhance smoothly. Follow-up 1 to 2 years without recurrence, the recent results were satisfactory. Conclusions This procedure is in line with the principle of tumor-free operation. It has the advantages of complete removal of the lesion and maximization of the maxillary function of the tumor. It is especially beneficial to the reconstruction and reconstruction and has certain clinical promotion value.
