目的探讨以精神障碍为首发症状的病毒性脑炎患者的临床特点及早期诊断。方法回顾性分析我院1999~2003年收治的46例以精神障碍为首发症状的病毒性脑炎患者的临床资料。结果46例患者经抗病毒治疗后,除1例起病2周后突然出现高热、昏迷的患者呈植物状态外,大多数患者预后良好。结论对以急性精神障碍为首发症状的患者,如果病前有感染、腹泻、过度疲劳、口唇疱疹等病史,在疾病的过程中出现智能减退、大小便失禁、发热、癫痫等症状,应尽快完善相关检查,动态观察病情,以明确诊断,指导治疗。
Objective To investigate the clinical features and early diagnosis of viral encephalitis in patients with mental disorders as the first symptom. Methods The clinical data of 46 patients with viral encephalitis who had mental disorder as the first symptom were retrospectively analyzed from 1999 to 2003 in our hospital. Results 46 cases of patients after antiviral therapy, except 1 case onset 2 weeks after the sudden onset of fever, coma in patients with plant status, the majority of patients with good prognosis. Conclusions Patients with acute mental disorder as the first symptom should develop as soon as possible if they have history of infection, diarrhea, over-fatigue and cold sores before onset of symptoms such as dementia, incontinence, febrile illness and epilepsy in the course of the disease Related checks, dynamic observation of the disease, to confirm the diagnosis and guide the treatment.
