目的探求发育性髋脱位(developmental dislocation of the hip,DDH)患儿与正常儿关节囊中Ⅰ、Ⅲ型胶原分布与含量的差异。方法对30例发育性髋脱位患儿及对照组12例小儿髋关节囊进行Ⅰ、Ⅲ型胶原免疫组化研究。并选取9对性别相同年龄相近的发育性髋脱位患儿与对照组配对比较。应用图像分析软件进行量化分析,并经统计学处理。结果关节囊滑膜层Ⅰ型胶原阳性染色强于纤维层,组间无明显差异;纤维层中Ⅰ型胶原阳性染色的成纤维细胞数占总计数成纤维细胞的百分比及Ⅰ型胶原染色的灰度值在髋脱位组与对照组间存在显著差异(P<0.05);全部标本中仅有3例大龄DDH患儿Ⅲ型胶原染色阳性。结论发育性髋脱位患儿关节囊中Ⅰ型胶原含量及合成能力均较正常同龄儿低,可能是导致髋关节囊松弛的原因。
Objective To investigate the distribution and content of type Ⅰ and type Ⅲ collagen in children with developmental dislocation of the hip (DDH) and normal children. Methods Thirty children with hip dysplasia and 12 children with congenital hip dysplasia were enrolled in this study. Type Ⅰ and Ⅲ collagen immunohistochemistry was performed on the hip joint capsule of children. Nine matched pairs of children with similar developmental dislocation of hip with similar sex were compared with the control group. Application of image analysis software for quantitative analysis, and the statistical processing. Results The positive staining of type Ⅰ collagen in the synovial membrane of the articular capsule was stronger than that of the fibrous layer. There was no significant difference between the two groups. The number of fibroblasts stained with type Ⅰ collagen in the fibrous layer accounted for the percentage of the total number of fibroblasts and type Ⅰ collagen-stained gray There was significant difference between the values of hip dislocation group and control group (P <0.05). Only 3 cases of type Ⅲ collagen in children with DDH were positive in all the specimens. Conclusions The content of collagen type Ⅰ and the ability of synthesis in joint capsule of children with developmental dislocation of hip are lower than those of normal peers, which may be the reason of relaxation of hip joint capsule.
