目的探讨胎盘生长因子(Pl GF)与胃癌发生和发展的关系。方法回顾性收集2013年3月至2014年12月期间在绵阳四〇四医院胃肠外科行胃癌根治术的88例胃癌患者作为研究对象,取其肿瘤组织(胃癌组)及癌旁组织(癌旁组织组)行免疫组织化学染色和逆转录-聚合酶链反应法(RT-PCR)检测Pl GF m RNA及其蛋白的表达,取其外周血行酶联免疫吸附法(ELISA)检测Pl GF的浓度。收集2014年9月于绵阳四〇四医院体检科行体检的30例健康体检者(体检组)的外周血行ELISA检测Pl GF的浓度。比较胃癌组和癌旁组织组组织中Pl GF m RNA及其蛋白的表达水平,比较胃癌组患者和体检组人员外周血中Pl GF的浓度,并分析胃癌组患者癌组织中Pl GF m RNA及其蛋白表达、外周血中Pl GF浓度与其临床病理学特征的关系。结果 RT-PCR结果显示:胃癌组组织中Pl GF m RNA的表达水平为0.569±0.166,高于癌旁组织的0.037±0.020(t=-29.948,P=0.000)。免疫组织化学染色结果显示:胃癌组组织中Pl GF的表达阳性率为80.7%(71/88),高于癌旁组织组的5.7%(5/88),差异具有统计学意义(χ2=100.867,P=0.000)。ELISA法检测结果显示:胃癌组患者外周血中的Pl GF浓度为(57.247±9.800)ng/L,高于体检组的(10.351±1.715)ng/L,差异有统计学意义(t=43.000,P=0.000)。胃癌患者癌组织中Pl GF m RNA及其蛋白的表达与肿瘤直径、p T分期、p N分期、分化程度及Borrmann分型均有关(P<0.050),肿瘤直径>4 cm、p T3~4期、p N3期、低分化及Borrmann分型为Ⅲ~Ⅳ型者Pl GF m RNA的表达水平和Pl GF的表达阳性率均较高;而Pl GF m RNA及其蛋白的表达与年龄、性别、p M分期及胃肠型均无关(P>0.050)。胃癌患者外周血中Pl GF的浓度与年龄、性别、肿瘤直径、p T分期、p N分期、p M分期、分化程度、Borrmann分型及胃肠型均无关(P>0.050)。结论胃癌患者癌组织及血液中Pl GF的表达异常可能与胃癌的发生和发展有关。
Objective To investigate the relationship between placental growth factor (PlGF) and the occurrence and development of gastric cancer. Methods A retrospective study was performed on 88 gastric cancer patients who underwent radical gastrectomy at the Gastrointestinal Surgery Center of 440 Hospital of Mianyang from March 2013 to December 2014. The patients with gastric cancer (gastric cancer group) and adjacent non-cancerous tissues The expression of PlGF m RNA and its protein was detected by immunohistochemical staining and reverse transcription-polymerase chain reaction (RT-PCR). Peripheral blood was collected for detection of Pl GF concentration. Peripheral blood samples of 30 healthy subjects (physical examination group) from September 2014 at the Physical Examination Hospital of 404 Hospital of Mianyang were collected to detect the concentration of PlGF. The levels of Pl GF m RNA and protein in gastric cancer group and adjacent non-cancerous tissue group were compared. The concentrations of Pl GF in peripheral blood of gastric cancer patients and physical examination group were compared. The levels of Pl GF m RNA, Its protein expression, the relationship between the concentration of Pl GF in peripheral blood and its clinicopathological features. Results The RT-PCR results showed that the expression level of Pl GF m RNA in gastric cancer tissue was 0.569 ± 0.166, which was higher than that in adjacent tissues (0.037 ± 0.020, t = -29.948, P = 0.000). Immunohistochemical staining results showed that the positive expression rate of PlGF in gastric cancer tissue was 80.7% (71/88), which was higher than that in adjacent non-cancerous tissues (5.7%, 5/88) (χ2 = 100.867 , P = 0.000). The result of ELISA showed that the concentration of Pl GF in peripheral blood of gastric cancer patients was (57.247 ± 9.800) ng / L, which was higher than that of the control group (10.351 ± 1.715) ng / L, the difference was statistically significant (t = 43.000, P = 0.000). The expression of Pl GF m RNA and its protein in gastric cancer was correlated with tumor diameter, p T stage, p N stage, differentiation degree and Borrmann type (P <0.050), tumor diameter> 4 cm, p T3 4 The expression level of Pl GF m RNA and the positive expression rate of Pl GF in patients with stage ¢ ó, stage ¢ ó N3, poorly differentiated and Borrmann type ¢ ó-¢ ó were higher than that in stage ¢ ó, while the expression of Pl GF m RNA and its protein was positively correlated with age, gender , P M stage and gastrointestinal type (P> 0.050). The concentration of PlGF in peripheral blood of patients with gastric cancer was not related to age, sex, tumor diameter, p T stage, p N stage, p M stage, differentiation degree, Borrmann type and gastrointestinal type (P> 0.050). Conclusion The abnormal expression of Pl GF in cancer tissues and blood of patients with gastric cancer may be related to the occurrence and development of gastric cancer.
