目的探讨系统性红斑狼疮(SLE)活动期患者Th细胞亚群平衡的改变及其意义。方法以趋化因子受体CCR5和CCR3为Th细胞亚群表面标记,利用三色流式细胞分析法对SLE患者外周血单个核细胞(PBMCs)中CD4+T细胞亚群平衡变化进行分析。结果活动期SLE患者外周血CD4+CCR5+CCR3- 细胞百分比与正常人比较差异无显著性(P>0.05),CD4+CCR5-CCR3+细胞百分比显著高于正常人(P<0.05), CD4+CCR5+CCR3- /CD4+CCR5-CCR3+比值显著低于正常人(P<0.05)。CD4+CCR5-CCR3+和(或)CD4+CCR5+CCR3-细胞百分比异常升高者出现组织脏器损伤的概率较高。肾上腺糖皮质激素治疗可使异常升高的CD4+CCR5-CCR3+和(或)CD4+CCR5+CCR3-细胞百分比有所降低。结论多数活动期SLE患者体内存在Th2细胞优势,Th2和(或)Th1细胞的改变与组织脏器损伤有一定的相关性。
Objective To investigate the changes of Th cell subsets and their significance in patients with active systemic lupus erythematosus (SLE). Methods The chemokine receptors CCR5 and CCR3 were used as the surface markers of Th cell subsets. The balance of CD4 + T cell subsets in peripheral blood mononuclear cells (PBMCs) from patients with SLE was analyzed by using three-color flow cytometry. Results The percentages of CD4 + CCR5 + CCR3- cells in peripheral blood of active SLE patients were not significantly different from those of normal controls (P> 0.05). The percentage of CD4 + CCR5-CCR3 + + CCR3- / CD4 + CCR5-CCR3 + ratio was significantly lower than normal (P <0.05). Patients with abnormally elevated CD4 + CCR5-CCR3 + and / or CD4 + CCR5 + CCR3- cells have a higher probability of developing organ tissue damage. Adrenal glucocorticoid treatment resulted in a decrease in the percentage of abnormally elevated CD4 + CCR5-CCR3 + and / or CD4 + CCR5 + CCR3- cells. Conclusion Th2 cells are predominant in SLE patients during most active periods, and the changes of Th2 and / or Th1 cells are related to the damage of tissues and organs.
