目的探讨正常新生儿乳酸的范围及脐动脉血乳酸水平与脐血血气的相关关系。方法对108例正常新生儿及55例诊断胎儿宫内窘迫的新生儿,在建立自主呼吸以前,分别采集脐动脉血、脐静脉血及末梢血,测定相应的乳酸值,并对临床诊断胎儿宫内窘迫的新生儿同时监测脐动脉血的血气分析。结果无临床胎儿宫内窘迫组新生儿脐动脉血平均乳酸值为(2.6±1.4)mmol/L,脐静脉血平均乳酸值为(2.3±1.2)mmol/L,末梢血的平均乳酸值为(2.8±1.3)mmol/L。脐动脉血与脐静脉血、末梢血的乳酸水平呈明显的直线相关;胎儿宫内窘迫组新生儿脐动脉血平均乳酸值为(4.3±2.4)mmol/L,脐静脉血平均乳酸值为(3.9±2.1)mmol/L,末梢血的平均乳酸值为(4.1±2.0)mmol/L;正常新生儿与宫内窘迫新生儿两组间的乳酸值比较差异有统计学意义(P<0.01)。结论新生儿脐动脉、脐静脉、末梢血的乳酸值具有明显的相关关系,乳酸检测可作为代谢性酸中毒的监测指标。
Objective To investigate the range of normal neonates lactic acid and the relationship between umbilical arterial blood lactate and umbilical blood. Methods 108 normal neonates and 55 neonates diagnosed with fetal distress were recruited. Before establishing spontaneous respiration, umbilical arterial blood, umbilical blood and peripheral blood were collected and the corresponding lactate levels were measured. Fetal Infarct Blood Gas Analysis Simultaneous Monitoring of Umbilical Artery Blood. Results The average umbilical artery lactate (2.6 ± 1.4) mmol / L, umbilical vein blood lactate (2.3 ± 1.2) mmol / L and the average lactate value of peripheral blood were ( 2.8 ± 1.3) mmol / L. Umbilical artery blood and umbilical venous blood, peripheral blood lactate levels were significantly linear; neonatal uterine fetus fetal distress group average lactate (4.3 ± 2.4) mmol / L, umbilical venous blood average lactate value ( 3.9 ± 2.1) mmol / L, and the average lactate level of peripheral blood was (4.1 ± 2.0) mmol / L. There was significant difference in lactic acid value between normal neonates and neonates with intrauterine distress (P <0.01) . Conclusion Neonatal umbilical artery, umbilical vein, peripheral blood lactate value has a clear correlation, lactic acid test can be used as metabolic acidosis monitoring indicators.
