目的探索新形势下社区卫生服务中心的全科医师在岗培训相关模式,并评价其培训效果。方法采用文献法、问卷法和测试法,采集在上海市长宁区医学会参加全科医学在岗培训和进入三级-二级-一级医院(3-2-1)分级协作平台培训的学员资料,通过构成比、频数分布和χ2检验来分析了解在岗培训学员的相关满意度和自身的目标诉求,而培训效果的相关性因素进一步采用Logistic回归进行分析,并比较分级协作平台培训模式和常规全科医学培训模式的学习效果。结果培训内容的可实践性得到了72.8%学员的首肯,社区中常见病多发病诊治以及急救培训是87.9%学员的兴趣目标,其次是全科医学的关联知识达到81.6%,病患的康复保健关注度是62.4%,对于公共卫生方面感兴趣的只有36.7%;而培训学员对老师带教业务水平和教材可读性的满意比例分别是66.2%、60.6%,在培训考核成绩方面,研究组(平台培训组)明显优于对照组(常规培训组),(83.00±6.75)vs.(67.00±6.02),P<0.05,有统计学意义;通过多因素回归分析学员的培训成绩,其结果与其年龄段相关性不明显,而与其学历和职称有一定的相关性。结论三级-二级-一级医院分级协作平台培训模式能得到社区卫生服务中心全科医师的认可,能一定程度地提高其培训效果,适合推广。
Objective To explore the on-the-job training mode of general practitioners in community health service centers under the new situation and to evaluate the training effects. Methods The literature, questionnaires and test methods were used to collect the information of trainees who participated in the training of general medical profession in Changning District Medical Association and the training of grading collaboration platform of Level 3 - Level 2 - Level 1 Hospital (3-2-1) , Through the composition ratio, frequency distribution and χ2 test to analyze and understand the related satisfaction of on-the-job trainees and their own target demands, and the correlation factors of training effect are further analyzed by Logistic regression, and compare the training mode of the hierarchical cooperation platform and the conventional Learning effect of medical training mode. Results The practicability of the training was approved by 72.8% of the participants. The diagnosis and treatment of frequently-occurring diseases and first aid training in the community were 87.9% of the participants’ interest targets, followed by 81.6% of the related knowledge in general practice. The rehabilitation of the patients The degree of concern is 62.4%, and only 36.7% are interested in public health. The satisfaction rate of the trainees in teaching the teachers’ teaching level and the readability of the materials is respectively 66.2% and 60.6%. In terms of training and examination results, the research group (P <0.05), which was significantly higher than that of the control group (normal training group), (83.00 ± 6.75) vs. (67.00 ± 6.02) respectively. The results of the training were analyzed by multivariate regression analysis And its age is not obvious relevance, but with their academic qualifications and titles have some relevance. Conclusion The training mode of three-level-two-level hospital hierarchical collaboration platform can be approved by the general practitioners of community health service center, which can improve the training effect to a certain extent and is suitable for promotion.
