目的本研究旨在运用ROC曲线对比分析TRUS和磁共振弥散加权成像(DWI)两种影像学方法对前列腺癌诊断的价值。方法选取行TRUS和DWI检查并有最终病理结果的患者53例。用SPSS 11.0软件对数据进行ROC曲线分析。对得出的ROC曲线下面积,使用Z检验。结果 22例PCa患者中有18例TRUS上表现为血流增多,Vmax>13cm/s,RI>0.68。19例PCa患者在DWI当b值为800s/mm~2时的ADC值范围为(0.68~1.41)×10~(-3)mm~2/s,平均(1.08±0.12)×10~(-3)mm~2/s。ROC曲线分析显示,ADWI=0.914,ATRUS=0.913。结论 TRUS和DWI均是PCa诊断准确率较高的有效方法。而TRUS相对价格低廉、操作方便,不仅可以用于PCa的筛查,还有着很高的灵敏度和相对较高的特异度。是一种临床首选的PCa影像学检查方法。
Objective The purpose of this study was to compare the diagnostic value of TRUS and diffusion-weighted imaging (DWI) imaging in the diagnosis of prostate cancer using ROC curves. Methods Totally 53 patients underwent TRUS and DWI examinations with final pathological findings. Data were analyzed by ROC curve using SPSS 11.0 software. For the area under the ROC curve, the Z test was used. Results Among the 22 patients with PCa, 18 showed increased blood flow on TRUS with Vmax> 13 cm / s and RI> 0.68. The ADC value of PCa patients with DWI when b value was 800 s / mm ~ 2 was 0.68 ~ 1.41) × 10 ~ (-3) mm ~ 2 / s, average (1.08 ± 0.12) × 10 ~ (-3) mm ~ 2 / s. ROC curve analysis showed ADWI = 0.914 and ATRUS = 0.913. Conclusion Both TRUS and DWI are effective methods for diagnosing PCa with high accuracy. The relative low price of TRUS, easy to operate, not only for PCa screening, but also has a high sensitivity and relatively high specificity. Is a clinically preferred method of PCa imaging.
