一般认为,交感神经系统的激活是心衰的一种代偿机制,并通过增加心搏量及升高血压维持循环。因而,β-阻滞剂在心衰的应用被认为是禁忌的,迄今所积累的实验及临床经验提示,血清儿茶酚胺升高对心功能可能有显著的损害作用。一些研究提出在心衰应用β-阻滞剂以图改善过度高感刺激的有害作用。本文讨论赞成及反对β-阻滞剂用于充血性心衰的基本原理,复习有关治疗中使用这些药物的临床经验。
Generally believed that the activation of the sympathetic nervous system is a compensatory mechanism of heart failure, and by increasing stroke volume and blood pressure to maintain the cycle. Therefore, β-blockers in the application of heart failure is considered to be taboo, so far accumulated experimental and clinical experience suggests that elevated serum catecholamines may have significant damage to cardiac function. Some studies suggest the use of beta-blockers in heart failure in an attempt to improve the detrimental effects of excessive hypersensitivity. This article discusses the rationale for favoring and opposing β-blockers for congestive heart failure and reviews clinical experience with the use of these drugs in the treatment.
