目的研究中国献血者中感染HBV的基因型及亚型的分布特征,分析献血者中HBV基因型、亚型与献血者HBV血清学特征及病毒学特征的关系。方法从2008~2010年5家合作血液中心(血站)HBsAg确证阳性献血者中随机选取245份样本进行研究;通过S区扩增测序结合多对型特异性引物巢式PCR法分析HBV基因型,通过PreS/S区扩增测序或全基因组扩增测序分析HBV基因亚型。用SPSS 11.0统计软件分析各地HBV基因型、基因亚型的分布,比较献血者中HBV基因型和亚型分布与慢性乙型肝炎患者的区别,分析HBV基因型、基因亚型与人口特征,病毒学特征和血清学特征的关系。结果 245份样本中成功确定基因型228例,成功确定基因亚型200例。5地献血者感染中HBV的主要基因型为B型和C型,B型流行率高于C型,未见E-H型;基因亚型依次为B2,C2,D1,A1;在昆明献血者中发现了B基因型的1种新亚型B9;C型和C2亚型中的HBeAg阳性率分别显著高于B型和B2亚型中的HBeAg阳性率。结论我国献血者中感染的HBV的主要基因型为B型和C型,B型高于C型,且具有流行率增高的趋势,C型献血者中HBV复制活性高于B型。C型中的主要亚型为C2,B型中的主要亚型为B2。
Objective To study the distribution characteristics of HBV genotypes and subtypes in Chinese blood donors and to analyze the relationship between HBV genotypes and subtypes in blood donors and HBV serology and virological characteristics of blood donors. METHODS: A total of 245 samples were randomly selected from 5 HBsAg positive blood donors from 5 collaborating blood centers (blood stations) from 2008 to 2010. The genotypes of HBV were analyzed by nested polymerase chain reaction (S-region PCR) and multiple pairs of specific primers The HBV genotypes were analyzed by PreS / S region amplification or whole genome amplification. The distribution of HBV genotypes and gene subtypes was analyzed by SPSS 11.0 statistical software. The differences between HBV genotypes and subtypes in blood donors and patients with chronic hepatitis B were compared. HBV genotype, genotype and population characteristics, The relationship between the features and serological features. Results A total of 228 samples of 245 samples were successfully genotyped and 200 were successfully identified. The main genotypes of HBV in the 5 donors were type B and C, the prevalence of type B was higher than that of type C, and no EH was found. The genotypes of genes were B2, C2, D1 and A1 in blood donors One new subtype B9 of B genotype was found; the positive rates of HBeAg in C and C2 subtypes were significantly higher than those in B and B2 subtypes, respectively. Conclusion The main genotypes of HBV infected in blood donors in our country are type B and type C, type B is higher than type C, and the epidemic rate is higher. The HBV replication activity in type C donors is higher than type. The major subtype in type C is C2, and the major subtype in type B is B2.
