目的探究地塞米松在肺炎合并胃酸吸入的急性肺损伤患者治疗中的治疗意义。方法选取2012年1月—2014年1月该院接收治疗的50例肺炎合并胃酸吸入的急性肺损伤患者,以及同期来该院体检的50例健康志愿者。50例肺炎合并胃酸吸入的急性肺损伤患者设为观察组使用地塞米松进行治疗。50例健康志愿者为对照组。观察组治疗结束后,采用双抗体夹心酶免疫吸附法(ABC-ELISA)对二组患者血清中细胞因子TNF-α、IL-4、IL-10的含量、GCs和GR水平进行测量和比较。并对二组患者的一般资料进行比较。结果研究结果显示二组患者的年龄、性别、吸烟史、饮酒史和体重指数等一般资料间差异无统计学意义(P>0.05)。观察组患者治疗后血清中细胞因子TNF-α、IL-4的含量较治疗前显著降低,差异具有统计学意义(P<0.05)。观察组患者治疗后血清中细胞因子IL-10的含量较治疗前患者显著升高,差异具有统计学意义(P<0.05)。观察组的GR水平显著低于对照组患者,GCs水平显著高于对照组。比较二组间差异具有统计学意义(P<0.05)。结论地塞米松在治疗肺炎合并胃酸吸入的急性肺损伤患者中疗效显著,可以在临床上进一步推广和使用。
Objective To investigate the therapeutic effect of dexamethasone in the treatment of acute lung injury patients with pneumonia and gastric acid inhalation. Methods From January 2012 to January 2014, 50 patients with acute lung injury with pneumonia and gastric acid inhalation admitted to the hospital and 50 healthy volunteers who were admitted to the hospital during the same period were selected. Fifty patients with acute lung injury with pneumonia and gastric acid inhalation were assigned to the observation group for treatment with dexamethasone. 50 healthy volunteers as control group. The levels of cytokines TNF-α, IL-4, IL-10, GCs and GR in the two groups were measured and compared by double antibody sandwich enzyme-linked immunosorbent assay (ABC-ELISA) And two groups of patients to compare the general information. Results The results showed that there was no significant difference between the two groups in general data such as age, sex, smoking history, alcohol consumption history and body mass index (P> 0.05). The levels of serum cytokines TNF-α and IL-4 in observation group were significantly lower than those before treatment (P <0.05). The serum level of IL-10 in the observation group was significantly higher than that before treatment, the difference was statistically significant (P <0.05). The level of GR in observation group was significantly lower than that in control group, GCs level was significantly higher than that in control group. The differences between the two groups were statistically significant (P <0.05). Conclusion Dexamethasone is effective in treating patients with acute lung injury complicated with pneumonia and gastric acid inhalation and can be further popularized and used clinically.
