目的探讨妊娠期糖尿病母婴血清及胎盘中内脂素(visfatin)的表达与胎儿生长发育的关系。方法采用酶联免疫吸附法及放免法分别测定30例妊娠期糖尿病(gestational diabetes mellitus,GDM)及30例正常孕妇母婴血清内脂素及胰岛素水平,同时测定空腹血糖、母婴体重及身高、计算胰岛素抵抗指数(HOMA-IR)及体重指数。采用逆转录-聚合酶链反应(RT-PCR)及免疫组织化学方法,检测两组胎盘内脂素基因mRNA的表达及蛋白的表达水平。结果①GDM组血清visfatin、FBG、FIN、HOMA-IR、BMI水平显著高于对照组(P<0.05)。②GDM组新生儿出生体重、RI、脐血visfatin、脐血血糖、脐血胰岛素及HOMA-IR水平显著高于对照组(P<0.05)。③GDM组胎盘组织visfatin mRNA及蛋白表达水平显著高于对照组(P<0.05)。结论 GDM孕妇血清及胎盘内脂素水平的升高可能参与了GDM的发生发展以及胎儿的宫内发育的调节。
Objective To investigate the relationship between the expression of visfatin and the growth and development of fetus in gestational diabetes mellitus and placenta. Methods The levels of serum visfatin and insulin in 30 gestational diabetes mellitus (gestational diabetes mellitus, gestational diabetes mellitus and 30 normal pregnant women) were determined by enzyme-linked immunosorbent assay (ELISA) and radioimmunoassay. The fasting blood glucose, body weight and height, Calculate insulin resistance index (HOMA-IR) and body mass index. RT-PCR and immunohistochemistry were used to detect the mRNA and protein expression of adiponectin in both groups. Results ① The levels of visfatin, FBG, FIN, HOMA-IR and BMI in GDM group were significantly higher than those in control group (P <0.05). (2) Neonatal birth weight, RI, visfatin, cord blood glucose, umbilical cord blood insulin and HOMA-IR in neonatal GDM group were significantly higher than those in control group (P <0.05). The level of visfatin mRNA and protein in placenta of GDM group was significantly higher than that in control group (P <0.05). Conclusions Elevated serum adiponectin level and placental lipid levels in GDM pregnant women may be involved in the development of GDM and the regulation of uterine development in the fetus.
