心功能不全所致的心排出量减少,通常发生在胸腹部大手术后。目前,以影响心肌收缩力治疗心衰的药物,包括儿茶酚胺类和毛地黄制剂。两者相比较,用毛地黄制剂的心排出量和心脏搏出指数仅稍高于儿茶酚胺类药物的心排出量和心脏搏出指数,故儿茶酚胺药物已在临床广泛使用。然而,曾有人报道,许多心血管损害与持续输注儿茶酚胺有关。输注儿茶酚胺可使心肌氧的供求系统解体,导致心肌损害,包括心肌纤维坏死和心内膜下出血。故儿茶酚胺用量应尽可能小,用药时间应尽可能短,以减少对心肌的损害。减少儿茶酚胺所致的缺氧损害的一个有效的方法
