目的:探讨山莨菪碱在心脏骤停心肺复苏早期的疗效。方法81例心脏骤停患者随机分为山莨菪碱组(n=44,心肺复苏时在常规综合治疗基础上给予山莨菪碱),对照组(n=37,给予生理盐水,其余用药两组相同)。治疗1,3,7天后比较两组患者的格拉斯哥昏迷评分(GCS)、胃黏膜 pH 值(pHi)、血乳酸值(BL)及中心静脉血氧饱和度(SCVO2)。结果山莨菪碱组复苏后第3天和第7天格拉斯哥评分明显高于对照组(均P <0.05);山莨菪碱组复苏后第1天、第3天和第7天血乳酸明显低于对照组(均P <0.05);胃黏膜pH值和中心静脉血氧饱和度明显高于对照组(均P <0.05)。结论对心脏骤停患者在常规治疗基础上加用山莨菪碱作为辅助用药有助于改善神经系统功能,降低乳酸水平,改善组织氧代谢及微循环状况,提高复苏效果。治疗后神经细胞功能恢复滞后于血液学指标的恢复。“,”Objective To explore the clinical effects of anisodamine (Ani) in the early stage of cardiopulmonary resuscitation. Methods A total of 81 cardiac arrest patients were randomly divided into two groups, anisodamine group (n=44) of which on top of the routine comprehensive treatment anisodamine was given and control group (n=37) of which only the routine comprehensive treatment and normal saline were given. Glasgow coma scale (GSC) score, blood lactic acid (BL), gastrointramucosal pH (pHi), and saturation of central venous oxygen (SCVO2) were examined on 1st , 3rd, and 7th day after the treatment. Results The GCS scores at the 3rd and 7th day after treatment, of anisodamine group reflected (10.2±1.4) and (12.4±1.6) respectively, both significantly higher than those of the control group [( 9.6±1.3) and (10.9±1.8) respectively, both P <0.05]. At the 1st, 3rd, and 7th day after treatment, the Bl values of anisodamine group were (3.12±1.35), (1.55±1.23), and (0.65±0.33), respectively, all significantly lower than those of the control group [(4.87±1.86), (2.77±1.52), and (1.46±0.78) respectively, all P <0.05]. The pHi values at the 1st, 3rd, and 7th day after treatment of anisodamine group were (0.73±0.36), (0.86±0.61), and (0.92±0.57) respectively, all significantly higher than those of the contyrol group [(0.65±0.53), (0.74±0.68), and (0.82±0.91) respectively, all P <0.05]. The SCVO2 values at the 1st, 3rd, and 7th day after treatment of anisodamine group were (7.27 ± 0.15), (7.35 ± 0.21), and (7.38 ± 0.19) respectively, all significantly higher than those of the control group [(7.11±0.31), (7.18±0.38), and (7.24±0.34), respectively, all P <0.05]. Conclusion Anisodamine application at the early period of resuscitation may improves oxygen metabolism and microcirculation of the tissues and promotes the function of nervous system. Thus it increases the effect of resuscitation, as well as the recovery of the function of neurons lags behind the recovery of hematological indexes.
