[目的]探讨 2 型糖尿病肾病患者血清胆红素(TB)及血尿酸(UA)水平的变化及其意义.[方法]于本院诊治的 2 型糖尿病患者,其中 30 例尿白蛋白排泄率(UAER)正常(UAER300 mg/24 h)为DN组;比较三组糖尿病病情、肾功能指标,血清TB以及UA水平及与肾功能指标的相关性.[结果]DN组 HbA1c 、空腹C肽均显著高于,糖尿病病程短于正常组和早期组(P <0.05);早期组 HbA1c 、空腹 C肽显著高于,糖尿病病程短于正常组(P <0.05);DN组 UAER、血清肌酐(SCr)均显著高于,内生肌酐清除率(CCr)显著低于正常组和早期组(P <0.05),早期组UAER、SCr显著高于,CCr显著低于正常组(P <0.05);DN组 UA 显著高于,TB 显著低于正常组和早期组(P <0.05 ),早期组 UA 显著高于,TB 显著低于正常组(P <0.05);血清TB与UAER、SCr、CCr呈负相关(P <0.05),UA水平与UAER、SCr、CCr呈正相关(P <0.05).[结论]DN发生发展过程伴随着血清TB降低以及 UA水平升高,其可用于 DN 早期辅助诊断及可能成为其临床治疗的新靶点.“,”[Objective]To investigate the changes and significance of serum bilirubin (TB)and uric acid (UA)levels in patients with type 2 diabetic nephropathy.[Methods]Among the type 2 diabetic patients diag-nosed and treated in our hospital,30 patients with normal urinary albumin excretion rate (UAER 300 mg/24h)were in the DN group.The condition of dia-betes mellitus,renal function index,serum TB and UA levels and their correlation with renal function index were compared among the three groups.[Results]HbA1c and fasting C-peptide in the DN group were signifi-cantly higher than those in the normal group and the early group (P <0.05);HbA1c and fasting C-peptide in the early group were significantly higher than those in the normal group (P <0.05);UAER and serum creati-nine (SCr)in the DN group were significantly higher than those in the normal group and the early group (P<0.05);and endogenous creatinine clearance rate (CCr)in the early group was significantly higher than those in the normal group and the early group (P <0.05).UA in the DN group was significantly higher than that in the normal group (P <0.05),TB in the DN group was significantly lower than that in the normal group and the early group (P <0.05),UA in the early group was significantly higher than that in the normal group (P<0.05),TB in serum was negatively correlated with UAER,SCr and CCr (P <0.05),UA level was posi-tively correlated with UAER,SCr and CCr (P <0.05).[Conclusion]The occurrence and development of type 2 diabetic nephropathy is accompanied by the decrease of serum TB and the increase of UA level,which can be used as a new target for early diagnosis and clinical treatment of DN.
