目的:探讨电子束CT三维血管造影(3D-EBCTA)在评价脑动静脉畸形手术前后中的价值与限度。材料和方法:选择11例不同部位、不同类型的脑动静脉畸形进行术前和术后的3D-EBCTA和 DSA对照研究,采用最大信号强度投影(MIP)和表面遮盖法(SSD)进行重建。结果:与DSA相比,术前3D-EBCTA对瘤巢、供血动脉(来源、数目及形态)和引流静脉的显示率分别为100%、100%和81.8%,术后瘤巢及供血动脉和引流静脉的处理情况显示满意率为100%。结论:3D-EBCTA无创、价廉,是诊断脑动静脉畸形的好方法,在术后随访方面可替代DSA,但在术前诊断上尚不能取代DSA。
Objective: To investigate the value and limitation of electron beam computed tomography (3D-EBCTA) in the evaluation of cerebral arteriovenous malformations before and after surgery. MATERIALS AND METHODS: Preoperative and postoperative 3D-EBCTA and DSA control studies were performed on eleven different sites and different types of cerebral arteriovenous malformations. MIP and SSD were used for reconstruction. Results: Compared with DSA, the preoperative 3D-EBCTA showed 100%, 100% and 81.8% of tumor nests, feeding arteries (source, number and morphology) and drainage veins, respectively The treatment of arteries and drainage veins showed a 100% satisfaction rate. Conclusion: 3D-EBCTA is a noninvasive and inexpensive method for the diagnosis of cerebral arteriovenous malformations. DSA can replace DSA in postoperative follow-up, but it can not replace DSA in preoperative diagnosis.
