多器官功能障碍综合征(MODS)病死率高达50%~90%,一般认为MODS是进入ICU的患者发病和病死的首要原因,但其发病机制迄今尚未阐明,感染特别是脂多糖及内毒素被认为是全身炎症反应综合征和MODS的主要启动因子。经多年的研究与探索,形成了从最早的器官缺血-再灌注损伤及感染学说到肠源性感染、微循环和全身炎症反应失控学说,还有学者提出细胞凋亡学说和非线性系统学说。这些理论的形成与血管内皮系统的结构、功能及其调节有密切关系,本文对在MODS过程中内皮细胞(EC)在凝血过程中的生理作用,EC受损与MODS凝血-抗凝血机制紊乱作一综述。
