甲氰眯胍引起急性肾功能衰竭(肾衰)罕见,致肝毒性作用亦少见,本文报道1例两次应用甲氰咪胍后均出现急性肾衰和碱性磷酸酶(ALK)显著升高。患者男性,67岁,1980年7月因呕血、便血,钡餐检查诊断为十二指肠溃疡用甲氰眯胍1g/d,病者血红蛋白(Hb)11.7g/d1,钠135mmol/L,尿素5.1mmol/L,肌酐1.1mg%,ALK13(KA)U。两个月后患者诉大量出汗和发晕,Hb8.5g/d1,钠123mmol/L,尿素19mmol/L,肌酐6.2mg%,ALK99(KA)U,疑药物中毒而停药,改用静注盐水而恢复。1981年患者诉上腹痛,内窥镜检查十二指肠溃扬,用甲氰咪胍1g/d治疗,当时 Hb 为9.7g/dl,尿素8.6mmol/L,肌酐
