目的分析单唾液酸四己糖神经节苷脂联合依达拉奉治疗急性脑梗死的疗效及对血清相关指标的影响。方法以2010年10月—2016年5月榆林市第一医院急诊救治的126例急性脑梗死患者为研究对象,根据就诊顺序分为观察组和对照组,每组63例。对照组患者在常规治疗基础上给予依达拉奉注射液,观察组在对照组的基础上加用单唾液酸四己糖神经节苷脂钠注射液,两组均连续治疗2周。比较两组的临床疗效,美国国立卫生研究院卒中量表(NIHSS)评分、Barthel指数评分,血清神经元特异性烯醇化酶(NSE)、S100β蛋白、氧化应激相关指标[超氧化物歧化酶(SOD)、晚期氧化蛋白产物(AOPP)和丙二醛(MDA)]水平在治疗前后的变化情况。结果观察组的总有效率(90.48%)显著高于对照组(76.19%),两组比较差异有统计学意义(P<0.05)。治疗2周后,两组的NIHSS和Barthel指数评分均较治疗前显著降低,同组治疗前后比较差异有统计学意义(P<0.05);且观察组NIHSS和Barthel指数评分显著低于对照组,差异有统计学意义(P<0.05)。两组的NSE、S100β蛋白、AOPP、MDA水平较治疗前显著降低,同组治疗前后比较差异有统计学意义(P<0.05);且观察组的以上指标显著低于对照组,差异有统计学意义(P<0.05);SOD水平较治疗前显著升高,同组治疗前后比较差异有统计学意义(P<0.05);且观察组的显著高于对照组,差异有统计学意义(P<0.05)。结论单唾液酸四己糖神经节苷脂可协同提高依达拉奉治疗急性脑梗死的临床疗效,促进患者神经功能及生活能力恢复,且发挥协同作用的机制可能与协同改变血清NSE、S100β蛋白、SOD、AOPP、MDA水平有关。
Objective To analyze the therapeutic effect of mono-sialic acid tetrahexose ganglioside combined with edaravone on acute cerebral infarction and its influence on serum related indexes. Methods A total of 126 patients with acute cerebral infarction who were treated in the First Hospital of Yulin City from October 2010 to May 2016 were selected as study subjects and divided into observation group and control group according to the order of treatment. Patients in the control group were given edaravone injection on the basis of routine treatment. The observation group was treated with monosialotetrahexosyl ganglioside sodium injection on the basis of the control group, and both groups were treated continuously for 2 weeks. The clinical efficacy of the two groups were compared. NIHSS score, Barthel index score, serum NSE, S100β protein and oxidative stress related index [superoxide dismutase (SOD, AOPP and MDA) before and after treatment. Results The total effective rate (90.48%) in the observation group was significantly higher than that in the control group (76.19%). There was significant difference between the two groups (P <0.05). After 2 weeks of treatment, the scores of NIHSS and Barthel index in both groups were significantly lower than those before treatment (P <0.05), and the scores of NIHSS and Barthel index in the observation group were significantly lower than those in the control group The difference was statistically significant (P <0.05). The levels of NSE, S100βprotein, AOPP and MDA in the two groups were significantly lower than those before treatment (P <0.05), and the above indexes in the observation group were significantly lower than those in the control group (P <0.05). The level of SOD was significantly higher than that before treatment, the difference was statistically significant before and after treatment (P <0.05), and the observation group was significantly higher than the control group, the difference was statistically significant (P < 0.05). Conclusions Monosialotetrahexosyl ganglioside can synergistically enhance the clinical efficacy of edaravone in the treatment of acute cerebral infarction and promote the recovery of neurological function and life ability in patients with acute cerebral infarction. The synergistic mechanism may be related to the synergistic effect of serum NSE, S100β protein , SOD, AOPP, MDA levels.
