目的:观察重组人粒细胞集落刺激因子(rhG-CSF)对健康造血干细胞捐献者脾脏大小的影响及其相关因素。方法:28例健康供者皮下注射rhG-CSF5μg/(kg.d),连续5d动员造血干细胞。在注射前和最后一次注射后第1、7及30天对健康供者进行脾脏超声检查,测量脾脏大小(长径×厚度);同时计数外周血中性粒细胞数量并检测细胞表面CD11b表达的相对平均荧光强度(MFI)。结果:供者脾脏的大小在注射前为(40.3±3.4)cm2,最后一次注射后第1、7和30天分别为(48.2±6.1)、(47.8±6.6)和(39.2±5.0)cm2,最后一次注射后第1、7天与注射前相比差异均有统计学意义(均P<0.013);供者外周血中性粒细胞数量在注射前为(3.4±1.0)×109/L,最后一次注射后第1和7天分别为(29.3±6.9)×109/L和(3.2±0.7)×109/L,最后一次注射后第1天与注射前相比差异有统计学意义(P<0.017);中性粒细胞表面CD11b表达的相对MFI在注射前为(57.1±43.5),最后一次注射后第1和7天分别为(96.8±51.3)和(66.9±30.4),最后一次注射后第1天与注射前相比差异有统计学意义(P<0.017)。线性回归分析结果显示,最后一次注射后第1天脾脏的增大与外周血中性粒细胞数量及细胞表面CD11b的表达均具有相关性(r=0.603,P=0.000;r=0.436,P=0.023)。结论:rhG-CSF可使健康供者脾脏在短期内明显增大,这与中性粒细胞数量以及细胞表面CD11b的表达相关。
Objective: To observe the effects of recombinant human granulocyte colony-stimulating factor (rhG-CSF) on the spleen size of healthy hematopoietic stem cell donors and its related factors. Methods: 28 healthy donors were given rhG-CSF 5μg / (kg.d) subcutaneously for 5 days to mobilize hematopoietic stem cells. Spleen ultrasonography was performed on healthy donors before and at 1, 7 and 30 days after the last injection to measure the size of the spleen (long diameter × thickness); meanwhile count the number of peripheral blood neutrophils and detect the expression of CD11b on the cell surface Relative average fluorescence intensity (MFI). Results: The size of donor spleen was (40.3 ± 3.4) cm2 before injection and (48.2 ± 6.1), (47.8 ± 6.6) and (39.2 ± 5.0) cm2 on the 1st, The difference between the 1st and the 7th day after the last injection was statistically significant (all P <0.013). The number of peripheral blood neutrophils in donor was (3.4 ± 1.0) × 109 / L before injection, (29.3 ± 6.9) × 109 / L and (3.2 ± 0.7) × 109 / L on the 1st and 7th day after the last injection, respectively. The difference between the first injection and the first injection after injection was statistically significant (P <0.017). The relative MFI of CD11b expression on neutrophils was (57.1 ± 43.5) before injection and (96.8 ± 51.3) and (66.9 ± 30.4) on the 1st and 7th day after the last injection, respectively There was a significant difference between the first day after injection and before injection (P <0.017). Linear regression analysis showed that the increase of spleen in the first day after the last injection was correlated with the number of peripheral blood neutrophils and the expression of cell surface CD11b (r = 0.603, P = 0.000; r = 0.436, P = 0.023). Conclusion: rhG-CSF can significantly increase the spleen of healthy donors in a short period of time, which is related to the number of neutrophils and the expression of cell surface CD11b.
