目的分析糖尿病患者进行血糖管理与缺血性卒中之间的关系,探讨糖尿病管理对缺血性卒中的影响。方法将186例(2009年5月-2011年5月)上海市徐家汇社区血糖库中的病例平均分为血糖、糖化血红蛋白干预组和非干预组。多元logistic回归筛选糖尿病造成缺血性卒中及影响缺血性卒中严重程度的危险因素。经过2年的干预比较两组发生缺血性卒中的情况。结果干预组通过对血糖,糖化血红蛋白的干预,发生卒中的可能性小,P值<0.05。非干预组发生卒中的可能性大,P值>0.05。尤其是糖化血红蛋白是影响缺血性卒中严重程度的危险因素。结论糖尿病与缺血性卒中关系密切,而且卒中后伴糖尿病进展快,预后差,病死率、残疾率及再发脑梗死率更高。通过干预糖化血红蛋白达到了减少缺血性卒中的发生率,提高患者的生活质量。
Objective To analyze the relationship between glycemic control and ischemic stroke in diabetic patients and explore the influence of diabetes management on ischemic stroke. Methods A total of 186 cases (from May 2009 to May 2011) of Shanghai Xujiahui community blood glucose database were divided into two groups: blood glucose, glycosylated hemoglobin intervention group and non-intervention group. Multivariate logistic regression was used to screen the risk factors for ischemic stroke and the severity of ischemic stroke in patients with diabetes mellitus. After two years of intervention, ischemic stroke occurred in both groups. Results Intervention group through the intervention of blood glucose, glycosylated hemoglobin, the possibility of stroke is small, P value <0.05. Non-intervention group the possibility of stroke, P value> 0.05. Glycated hemoglobin, in particular, is a risk factor for the severity of ischemic stroke. Conclusions Diabetes is closely related to ischemic stroke, with rapid progression of stroke and poor prognosis after stroke, with a higher case fatality rate, disability rate and recurrent cerebral infarction rate. Through the intervention of glycosylated hemoglobin to reduce the incidence of ischemic stroke and improve the quality of life of patients.
