目的评价优化脉冲技术(Optimal Pulse Technology,OPT)、脉冲染料激光(Pulsed dye laser,PDL)和连续Nd∶YAG激光联合治疗婴幼儿鲜红斑痣的疗效与安全性。方法鲜红斑痣患儿32例,年龄3个月至7岁,先经OPT治疗2次,再用PDL治疗3次,共经过5次治疗,治疗间隔2~3个月。其中5例伴有疣状增生患儿,联合应用连续Nd∶YAG激光治疗。观察临床疗效与不良反应的发生情况。结果 32例鲜红斑痣患儿,痊愈9例,痊愈率28.1%;显效18例,显效率56.3%,总有效率84.4%;5例伴疣状增生者均1次治愈,治愈率100.0%。比单纯应用PDL治疗,疗效有所提高。32例患者中,出现轻度色素减退3例(占9.4%),出现色素沉着2例(6.3%),无瘢痕形成。结论 OPT、PDL治疗婴幼儿鲜红斑痣疗效可靠、并发症少,连续Nd∶YAG激光可辅助治疗有疣状增生的病变。
Objective To evaluate the efficacy and safety of Optimal Pulse Technology (OPT), pulsed dye laser (PDL) and continuous Nd: YAG laser in the treatment of port wine stains in infants. Methods 32 cases of port wine stains children, aged 3 months to 7 years, first treated with OPT twice, and then treated with PDL 3 times a total of 5 treatments, the treatment interval of 2 to 3 months. Five of them were accompanied by verrucous hyperplasia and were treated with continuous Nd:YAG laser. To observe the clinical efficacy and adverse reactions occurred. Results In 32 cases of port wine stains, 9 cases were cured and the cure rate was 28.1%. 18 cases were markedly effective, the effective rate was 56.3% and the total effective rate was 84.4%. The 5 cases with verrucous hyperplasia were cured once and the cure rate was 100.0%. Than the simple application of PDL treatment, efficacy has improved. Among the 32 patients, mild hypopigmentation was found in 3 (9.4%), hyperpigmentation in 2 (6.3%), and no scarring. Conclusion OPT and PDL are reliable and have fewer complications in the treatment of port wine stains. Continuous Nd:YAG laser can be used to treat the lesions with verrucous hyperplasia.
