目的减少尿道下裂手术后并发症的发生，提高手术成功率，取消尿道代用物。方法选择尿道下裂Ⅰ型和部分Ⅱ型患者。26例行尿道松解延长电头尿道口成形术，14例行尿道松解延长电头隧道法尿道口成形术。24例留置尿管，16例尿流改道，二种方法术后均于1wk拔管。结果40例术后阴茎龟头畸形完全矫正，恢复后外观正常，仅1例因感染引起漏尿，2例尿道口狭窄。结论尿道松解延长龟头尿道口成形术为自身尿道的延长，成功率高，不需尿道代用物，恢复快，明显减少术后并发症，对于轻型尿道下裂是种理想的手术方法。
Objective To reduce the incidence of complications after hypospadias surgery, improve the success rate of surgery and eliminate the urethral surrogate. Methods Select hypospadias type Ⅰ and type Ⅱ patients. Twenty-six cases underwent urethral loosening and extension electrotome, and 14 cases underwent urethroplasty with urethral loosening and extension electrical head tunneling. 24 cases of indwelling catheter, 16 cases of urinary diversion, two methods were extubated in 1wk. Results 40 cases of postoperative glans penis deformity completely corrected, the appearance of normal after recovery, only 1 case of urine leakage caused by infection, 2 cases of urethral stenosis. Conclusions Urethral loosening and prolongation of glans urethroplasty is an extension of the urethra. The success rate is high. No urethral substitutes are needed, the recovery is quick, and the postoperative complications are obviously reduced. It is an ideal surgical method for the light hypospadias.
