目的探讨99Tcm-DTPA肾动态显像测得肾小球滤过率(GFR)结果与不同病理类型的IgA肾病(IgAN)组织病变程度的相关性。方法选择经肾活检穿刺病理证实为IgA肾病的患者78例,对其肾小球、肾小管间质和血管损害程度行Katafuchi’s积分和Lee氏分级,全部患者行99Tcm-DTPA肾动态显像肾小球滤过率(GRF)测定。结果肾小球滤过率测定值与肾小球、肾小管间质和血管损害程度积分呈负相关。结论99Tcm-DTPA肾动态显像测得肾小球滤过率结果与肾活检病理损害程度有明显相关性,肾间质、肾小管病变严重者肾小球滤过率值明显下降,血肌酐在正常范围内但已有肾小球滤过率下降者对预后判断具有实用价值。
Objective To investigate the relationship between the glomerular filtration rate (GFR) and the pathological changes of IgA nephropathy (IgAN) in 99Tcm-DTPA renal dynamic imaging. Methods Seventy-eight patients with pathologically confirmed IgA nephropathy were enrolled in this study. Katafuchi’s score and Lee’s score were used for glomeruli, tubulointerstitial and vascular damage. All patients underwent 99Tcm-DTPA renal dynamic imaging Ball Filtration (GRF) Determination. Results The values of glomerular filtration rate were negatively correlated with the scores of glomerular, tubulointerstitial and vascular damage. Conclusion 99Tcm-DTPA renal dynamic imaging results of glomerular filtration rate and renal biopsy showed a significant correlation between the degree of pathological damage, renal interstitium, renal tubular lesions in severe glomerular filtration rate was significantly decreased, serum creatinine in Within the normal range but the glomerular filtration rate has decreased the prognostic judgment has practical value.
