目的 探讨放射过程中增殖细胞核抗原 (PCNA )的变化与宫颈癌放射敏感性的关系。方法 采用免疫组化ABC法检测 5 2例宫颈癌患者 2次后装治疗前后PCNA的表达情况 ,并分析PCNA改变与肿瘤体积缩小、放射损伤程度以及放疗结束后宫颈肿瘤控制情况的关系。结果 以PCNA下降的平均值 10 %为临界 ,把病例分为PCNA下降≥ 10 %组 (A组 )和PCNA下降 <10 %组 (B组 ) ,A组肿瘤体积缩小率为 76 .0 %± 13 .0 % ,B组为 46 .0 %± 33 .0 % ,P <0 .0 1;A组放射性损伤分级为Ⅱb~Ⅲ级的患者占 76 .0 % ,B组为 2 5 .9% ,P <0 .0 1;A组放疗后宫颈肿瘤控制率为 84.0 % ,B组为 74.1% ,P >0 .0 5 ;A组放疗前PCNA的表达率为 6 1.1%± 14 .8% ,B组为 72 .7%± 18.6 % ,P <0 .0 5。结论 放疗初始PCNA的变化可能是预测宫颈癌放射敏感性及疗效的新指标 ,但尚须积累资料作进一步验证。
Objective To investigate the relationship between the changes of proliferating cell nuclear antigen (PCNA) and radiosensitivity of cervical cancer during radiation. Methods Immunohistochemical ABC method was used to detect the expression of PCNA before and after 2 times dressing treatment in 52 cases of cervical cancer patients. The relationship between PCNA changes and tumor volume reduction, radiation injury and cervical cancer control after radiotherapy were analyzed. Results The mean reduction of PCNA was 10%. The cases were divided into 10% reduction in PCNA group (group A) and 10% reduction in PCNA group (group B). The reduction rate of tumor volume in group A was 76.0% ± 13.0% in group B, 46.0% ± 33.0% in group B, P <0.01; 76.0% in group A with grade Ⅱb-Ⅲ, and 25.9 in group B %, P0.01; The control rate of cervical cancer in group A after radiotherapy was 84.0%, in group B 74.1%, P> 0.05; the expression rate of PCNA before radiotherapy in group A was 61.1% ± 14.8 % In group B, 72.7% ± 18.6% in group B, P <0.05. Conclusions The change of initial PCNA in radiotherapy may be a new index to predict the radiosensitivity and curative effect of cervical cancer. However, it is necessary to accumulate data for further verification.
