目的探讨P16、Bcl-2、Ki-67、P53、ER、PR在子宫平滑肌肿瘤中的表达及临床意义。方法选取2011年3月-2015年12月唐山市开平医院收治的50例子宫平滑肌肿瘤患者作为观察组,另选取50例体检健康志愿者作为对照组,采用免疫组织化学二步法测定两组的P16、Bcl-2、Ki-67、P53、ER、PR 6个蛋白的表达情况。结果观察组P16、Bcl-2、Ki-67和P53阳性率明显高于对照组(P<0.05);两组ER以及PR阳性率相比,差异无统计学意义(P>0.05)。P16、Bcl-2、Ki-67和P53对正常子宫肌层与良性子宫肌瘤的鉴别差异有统计学意义(P<0.05),其中Ki-67的鉴别价值最高,其次为P53、Bcl-2以及P16;ER、PR的灵敏度和特异度较低(P>0.05)。结论 P16、Bcl-2、Ki-67、P53 4个蛋白的高表达提示与子宫平滑肌瘤密切相关,可以作为子宫平滑肌肿瘤诊断的重要指标,尚未发现ER、PR与子宫平滑肌肿瘤之间的关系。
Objective To investigate the expression and clinical significance of P16, Bcl-2, Ki-67, P53, ER and PR in uterine smooth muscle tumors. Methods Fifty patients with uterine smooth muscle tumors treated in Kaiping Hospital of Tangshan from March 2011 to December 2015 were selected as observation group and 50 healthy volunteers were selected as control group. Immunohistochemical two-step P16, Bcl-2, Ki-67, P53, ER, PR six protein expression. Results The positive rates of P16, Bcl-2, Ki-67 and P53 in the observation group were significantly higher than those in the control group (P <0.05). There was no significant difference in the positive rates of ER and PR between the two groups (P> 0.05). The difference of P16, Bcl-2, Ki-67 and P53 between normal myometrium and benign uterine fibroids was statistically significant (P <0.05), of which Ki-67 had the highest differential value, followed by P53, Bcl-2 And P16; the sensitivity and specificity of ER and PR were lower (P> 0.05). Conclusion The high expression of 4 proteins of P16, Bcl-2, Ki-67 and P53 is closely related to uterine leiomyomas and may be used as an important index in the diagnosis of uterine smooth muscle tumors. The relationship between ER, PR and uterine smooth muscle tumors has not been found .
