掌指关节处伸指肌腱脱位,常在类风湿性关节炎的病人中见到.而急性创伤性脱位则十分罕见.本文报告1例如下.患者男,27岁.在一次搬运重物时,右手中指伸直位过度尺偏位损伤;当即觉局部痛,伸直困难.受伤1天后就诊,检查见右中指下垂畸形,掌指关节背侧轻肿、压痛,掌指关节层曲时,肌腱滑向掌骨尺侧并疼痛.诊断为急性创伤性伸指肌腱脱位.在局麻下做了修补术;术中见伸肌腱挠侧筋膜有一长约2 cm纵行裂口,缝合该裂口,肌腱即复位.术后伸直位固定3周,3个月后随诊,患指伸屈正常.
