目的 对正常CS的CT和MRI序列比较研究 ,探讨CS实用影像检查方法。方法 6 0例鞍区及CS正常的病人分为三组 :① 2 0例行SET1WI、FSET2 WI、脂肪抑制SET1WI、GRET1WI和SET1WI增强扫描 ;② 2 0例行 3DSPGR和高分辨FSET2 WI;③另 2 0例行冠状CT增强扫描。采用双盲法评价并比较 (秩和检验 ) 8种检查方法对CS区正常解剖结构的显示能力和伪影程度。结果 ( 1)显示CS内第Ⅲ、Ⅴ1和Ⅵ对颅神经 :SET1WI增强扫描和CT增强扫描优于其他 6种序列 (Ρ <0 .0 5 ) ;( 2 )显示CS边缘 :GRET1WI和3DSPGR不如其他 6种方法 (Ρ <0 .0 5 ) ;( 3)显示CS间隙 :GRET1WI、3DSPGR和高分辨FSET2 WI不如其他 5种方法 (Ρ <0 .0 5 ) ;( 4 )显示ICA :GRET1WI、3DSPGR、高分辨FSET2 WI和CT增强扫描不如其他 4种方法 (Ρ <0 .0 5 ) ;( 5 )显示骨结构 :CT优于MRI各序列(Ρ <0 .0 5 ) ;( 6 )磁敏感伪影 :GRET1WI、3DSPGR和高分辨FSET2 WI多于其他序列 (Ρ <0 .0 5 ) ;( 7)ICA相位方向伪影 :GRET1WI和3DSPGR多于其他序列 (Ρ <0 .0 5 )。结论 CS区首选检查为冠状位SET1WI和FSET2 WI ;脂肪抑制SET1WI和SET1WI增强扫描可在必要时使用 ;疑有骨结构改变时 ,需使用CT或CT增强扫描 ;GRET1WI和 3DSPGR不适于CS区检查
Objective To compare the CT and MRI sequences of normal CS and to explore the CS practical imaging method. Methods Sixty patients with normal sellar region and CS were divided into three groups: ①20 patients underwent SET1WI, FSET2 WI, fat suppression SET1WI, GRET1WI and SET1WI enhanced scans; ②20 routine 3DSPGR and high resolution FSET2 WI; 2 0 routine coronary CT enhanced scan. Double blind method was used to evaluate and compare the display ability and degree of artifact of the normal anatomical structures of CS area by eight kinds of examination methods (rank sum test). Results (1) showed that CS, Ⅴ1 and Ⅵ in CS were better than cranial nerves in SET1WI enhanced CT and contrast-enhanced CT in other 6 kinds of sequences (P <0.05); (2) CS edges were shown: GRET1WI and 3DSPGR were not as good (6) The ICA: GRET1WI, GRET1WI, GRAP1WI, GRAP1WI, GRAP1WI, 3DSPGR, high-resolution FSET2 WI, and CT enhanced scanning were inferior to the other 4 methods (P <0.05); (5) Bone structures were shown: CT was superior to MRI sequences (P <0.05); (6) Sensitive artifacts: GRET1WI, 3DSPGR and high-resolution FSET2 WI were more than other sequences (P <0.05); (7) ICA phase-direction artifacts: GRET1WI and 3DSPGR more than other sequences (P <0.05). CONCLUSIONS: The coronal SET1WI and FSET2 WI are the best candidates for CS. Fat-suppressed SET1WI and SET1WI enhanced scans may be used if necessary. CT or CT-enhanced scans should be used if skeletal structure changes are suspected. GRET1WI and 3DSPGR are not suitable for CS
