目的应用微波加热治疗被肿瘤侵及的下颌骨,探讨近期疗效和并发症。方法10例口腔恶性肿瘤侵及或与下颔骨粘连,采用常规手术方式扩大切除软组织病变。切除癌瘤侵及或粘连的下颌骨部分,但保留下颔骨的连续性。标记需要处理的骨段,用钢网将其他组织保护。应用微波辐射器探头有效区域加热目标骨段。用电子测温仪检测加热区及保护区温度,使得温度维持在55℃,保持20分钟。术后常规关闭伤口。放置负压引流,预防性抗感染治疗。1、3和6个月行影像学检测。结果患者术后局部肿胀不明显.引流量不多,未出现明显疼痛。7例伤口愈合良好。2周正常出院;3例患者在术后2周时出现口内伤口裂开,下颌骨组织暴露。约l-2月时经热疗处理过的骨组织发生病理性骨折。所有患者经随访未见肿瘤复发。结论微波加热处理被肿瘤侵及的下颌骨骨段近期疗效尚可。只要严格选择适应证,无明显副作用,远期疗效有待于进一步观察。
Objective To apply microwave heating to treat the mandible invading the tumor and discuss the short-term curative effect and complications. Conclusion The microwave treatment of mandibular bone invaded by tumor is effective in short term. As long as the strict selection of indications, no obvious side effects, long-term efficacy remains to be further observed.
