目的探讨多层螺旋CT诊断下腔静脉后输尿管方法和临床价值。方法对12例经B超或排泄性泌尿系造影拟诊为下腔静脉后输尿管的患者进行多层螺旋CT泌尿系造影检查。按动脉期、静脉期、排泄期三期扫描,充分显示肾脏、输尿管、血管情况。根据动脉期、静脉期显影情况及对扩张段输尿管进行单层预扫描,把握输尿管造影的最佳时机,并将原始图像进行多种方法后处理重建。结果12例患者均显影良好,诊断准确,可清晰显示下腔静脉与输尿管的关系,与手术所见完全相符。结论掌握好输尿管的显影时机,采取合理体位,充分运用工作站的的图像后处理功能,多层螺旋CT完全可以对下腔静脉后输尿管作出立体、直观、准确的诊断。
Objective To investigate the method and clinical value of multi-slice spiral CT in the diagnosis of posterior inferior vena cava ureter. Methods Twelve cases of ureteral calculi diagnosed as inferior vena cava by B-ultrasound or excretion urography were examined by multi-slice spiral CT urography. According to the arterial phase, venous phase, excretion phase three scan, fully demonstrated the kidney, ureter, vascular conditions. According to the arterial phase, venous phase of development and expansion of ureteral single-layer pre-scan to grasp the best timing of ureterography, and the original image after a variety of methods of post-processing reconstruction. Results All the 12 patients developed well and the diagnosis was accurate. The relationship between inferior vena cava and ureter was clearly shown, which was in good agreement with the surgical findings. Conclusions Grasp the development timing of ureter, take a reasonable position, make full use of the workstation image post-processing function, multi-slice spiral CT can make three-dimensional, visual and accurate diagnosis of the inferior vena cava ureter.
