目的:观察氯磷定持续静脉维持和重复静脉给药对急性重度有机磷中毒的疗效。方法:采用前瞻性研究方法,将37例患者随机分为2组,均应用机械通气,静脉给予负荷量氯磷定后,对照组18例重复静脉给药;治疗组19例8 mg·kg-1.h-1微泵静脉维持。2组其他治疗相同。结果:治疗组阿托品用量和使用时间、机械通气时间明显低于对照组(P<0.01);治疗3,7 d后全血ChE活性明显高于对照组。结论:氯磷定持续静脉维持可加速急性重度有机磷中毒患者全血ChE的恢复,减少阿托品的用量和用药时间,减少机械通气时间。
Objective: To observe the curative effect of continuous intravenous administration of chlorophosphorus and repeated intravenous administration on acute severe organic phosphorus poisoning. Methods: A prospective study was conducted in which 37 patients were randomly divided into 2 groups. All of them were given mechanical ventilation. After intravenous injection of chlorophosphonamid, 18 patients in the control group received repeated intravenous administration. In the treatment group, 19 patients (8 mg · kg- 1.h-1 micro-pump venous maintenance. The other 2 groups were the same. Results: The dosage and time of atropine in the treatment group and the duration of mechanical ventilation were significantly lower than those in the control group (P <0.01). After 3 and 7 days of treatment, the activity of ChE in whole blood was significantly higher than that of the control group. CONCLUSION: Continuous maintenance of CPP can accelerate the recovery of whole blood ChE in patients with acute severe organophosphorus poisoning, reduce the dosage of atropine and medication time, and reduce the duration of mechanical ventilation.
