目的:探讨集束化干预策略在预防高原小儿呼吸机相关性肺炎(VAP)中的应用。方法:对2012年1月—3月收住我科的54例机械通气患儿进行集束化护理的前瞻性研究,所有患儿均采取头高位、吸痰时严格无菌操作、严格执行手卫生、定期更换呼吸机管道、加强口腔护理、及时倾倒接水杯等集束化护理措施。观察其体温、血象、胸部X线片及痰培养。选取我科2011年1月—3月行机械通气的41例患儿作对照。结果:采取集束化干预策略护理后,患儿VAP的发生率明显下降。结论:集束化干预策略护理能减少机械通气患儿肺部感染的概率,缩短住院天数,从而降低医疗费用。
Objective: To explore the application of cluster intervention in the prevention of pneumonia in high altitude ventilator-associated pneumonia (VAP). Methods: A prospective study was conducted on 54 cases of mechanical ventilation in our department from January to March in 2012. All children were treated with head high, strict aseptic operation during sputum aspiration, and hand hygiene , Regular replacement of ventilator pipeline, strengthen oral care, timely dump cups and other cluster care measures. Observe the body temperature, blood, chest X-ray film and sputum culture. Select our department in January 2011 March line of mechanical ventilation in 41 children as a control. Results: After the cluster nursing intervention, the incidence of VAP in children decreased significantly. CONCLUSION: Cluster nursing intervention can reduce the probability of pulmonary infection in children with mechanical ventilation and shorten the length of hospital stay, thus reducing the medical costs.
