目的 了解三种切口白内障摘除及人工晶体植入术后角膜形态改变及手术性散光的差异。方法 用TMS—2 型角膜地形图仪记录11m m (A 组)、6m m (B组)、3.5m m (C组)三种切口白内障摘除及人工晶体植入术的46 例50 只眼术前及术后3 个月角膜地形图变化。结果 A 组术后地形图均为蝴蝶结形,纵向占86% ,B组与C组75% 为蝴蝶结形,其中B组纵向占53% ,C组全部为水平向。三组均产生手术性散光,A 组明显大于B、C组,而B组与C组间差异不显著。结论 三种切口术后角膜地形图均发生改变,切口长度对术后角膜形态改变和手术性散光有显著影响,术后3 个月6m m 与3.5m m 切口造成的手术性散光无显著差异
Objective To investigate the changes of corneal morphology and surgical astigmatism after three types of incision cataract extraction and intraocular lens implantation. Methods 46 patients (50 eyes) with cataract extraction and intraocular lens implantation under the conditions of 11 m m (group A), 6 m m (group B) and 3.5 mm (group C) incision were recorded with TMS-2 corneal topography instrument And 3 months after the change of corneal topography. Results The postoperative topographic maps of group A were all bowknot, with 86% of the length in the longitudinal direction and 75% of the bowel shape in the group B and C, of which 53% were longitudinally in group B and all were horizontal in group C. All three groups had operative astigmatism, A group was significantly larger than B, C group, while B group and C group was not significantly different. Conclusions Corneal topography changes after the three kinds of incision. The length of the incision has significant effects on the morphological changes of the cornea and the surgical astigmatism. There is no significant difference in surgical astigmatism caused by 6 m m and 3.5 m m incisions at 3 months after operation
