目的分析急性心肌梗死(AMI)伴高血压患者的临床特征及卡托普利的影响。方法14962例AMI患者随机分为卡托普利组或安慰剂组,分析伴有或无高血压患者的临床特点。结果AMI伴有高血压史者6046例,占总人数的40.4%,男性占69.4%。有高血压病史(HHT)与无高血压史(NHT)者平均年龄分别为62.8±9.3,60.2±11.3岁;合并糖尿病史者分别为12.8%,7.0%;合并陈旧性心肌梗死史者分别为14.1%,11.0%;基础收缩压分别为136.8±26.2,119.4±20.1mmHg;舒张压分别为86.4±15.7,77.8±12.7mmHg;随机治疗前心力衰竭发生率分别为24.6%,19.4%。HHT、NHT病人卡托普利治疗组与安慰剂组的基础临床特征相似。随机治疗4周,HHT治疗组28d病死率(10.3%)较安慰剂组(11.6%)减少11.2%(P=0.12),NHT(8.2%vs8.5%)减少3.5%(P=0.68);卡托普利组与安慰剂组比较,HHT、NHT心力衰竭发生率(19.4%vs21.1%;15.5%vs16.9%)分别减少8.1%(P=0.09),8.3%(P=0.07);心室颤动(3?
Objective To analyze the clinical characteristics of patients with acute myocardial infarction (AMI) accompanied with hypertension and the influence of captopril. Methods A total of 14962 AMI patients were randomly divided into captopril group or placebo group, and the clinical features of patients with or without hypertension were analyzed. Results There were 6,046 AMI patients with history of hypertension, accounting for 40.4% of the total number of men and 69.4% of men. The average age of people with history of hypertension (HHT) and without history of hypertension (NHT) was 62.8 ± 9.3 and 60.2 ± 11.3 years old, respectively; those with history of diabetes mellitus were 12.8% and 7 respectively. 0% respectively; the history of AMI was 14.1% and 11.0% respectively; the systolic blood pressure was 136.8 ± 26.2 and 119.4 ± 20.1mmHg respectively; the diastolic blood pressure was 86.4 ± 15.7, 77.8 ± 12.7mmHg; incidence of heart failure before randomization were 24.6%, 19.4%.
