目的:探讨鼻咽癌侵犯海绵窦的MRI特点。材料与方法:搜集病理确诊的鼻咽癌伴有前组颅神经损害为主要临床症状的MRI资料33例。采用ElscintGyrexⅤ型磁共振仪,0.5T,超导型。SE,横断位,T1WI(440/25ms),PWI(2200/20ms),T2WI(2200/90ms),计33例;其中17例加扫冠状位,T1WI(440/25ms);10例进行了增强MRI扫描。结果:(1)两侧海绵窦不对称,左17例,右11例,双侧5例;(2)受累海绵窦呈局限或弥漫性增厚23例;(3)颈内动脉受累13例,颅神经受累32例,视神经受累7例;(4)海绵窦异常强化10例,脑膜异常强化5例;(5)骨质破坏:蝶窦14例,筛窦7例,眶尖与蝶嵴5例,颅底27例;(6)鼻咽部原发病灶33例。结论:MRI可精确显示鼻咽癌侵犯海绵窦的部位、途径、范围和程度,是优良诊断方法。
Objective: To investigate the MRI features of nasopharyngeal carcinoma invading the cavernous sinus. MATERIALS AND METHODS: 33 cases of pathologically confirmed nasopharyngeal carcinoma with cranial nerve damage in the anterior group were selected as the main clinical symptoms. ElscintGyrex V-type magnetic resonance instrument, 0.5T, superconducting type. SE, transverse position, T1WI (440 / 25ms), PWI (2200 / 20ms) and T2WI (2200 / 90ms) MRI scan. Results: (1) asymmetric cavernous sinus on both sides of the left 17 cases, right in 11 cases, bilateral in 5 cases; (2) involvement of cavernous sinus was limited or diffuse thickening of 23 cases; (3) 13 cases of internal carotid artery involvement , Cranial nerve involvement in 32 cases, optic nerve involvement in 7 cases; (4) cavernous sinus anomaly in 10 cases, meningeal anomaly in 5 cases; (5) bone destruction: sphenoid sinus in 14 cases, ethmoid sinus in 7 cases, 5 cases, 27 cases of skull base; (6) 33 cases of primary nasopharyngeal lesions. Conclusion: MRI can accurately show the site, path, extent and extent of nasopharyngeal carcinoma invading the cavernous sinus, and is an excellent diagnostic method.
