患者,男性,73岁,因患前列腺增生症,服用己烯雌酚2mg,每日3次,用药后排尿困难减轻。用药第7天患者出现精神异常:脱掉裤子,头顶被盖到处走动,行走不稳,烦燥不安,自言自语,食品、物品到处乱放。考虑为己烯雌酚引起,故停用。第2天患者精神异常消失,神志清楚,经询问,对昨日发生的事情有印象,但当时无法控制自己的行为。检查患者的肝肾功能均正常。以往无类似病史。己烯雌酚常用于治疗前列腺增生症,并有一定疗效。副作用主要是消化道症状,少见精神症状。本例出现的精神异常,可能是患者年龄大,肝肾功能减退,加上药物剂量偏大,在体内不能完全灭活,作用于中枢神经系统而引起精神异常;或者是己烯雌酚引起了内分泌功能紊乱,体内激素水平失去平
