目的评估临床用血电子评估监控系统对规范临床合理用血的成效。方法本院自2013年1月推行用血前后电子评估监控系统,2015年1月推行输血过程监控系统。统计分析2012-2015年手术量,围手术期用血情况,2013-2015年输血不良反应发生情况。结果 2012-2015年手术量持续增长,但围手术期平均用血量却呈下降趋势,2013-2015年血液出库至输血开始间隔平均时间分别为34.38±11.15,35.35±10.56和28.21±5.262 min;2015年间隔时间较前2年显著缩短,P<0.05;2013-2015年2 h内输血反馈率分别为41.2%,43.5%和72.3%,2015年较前2年反馈及时率明显升高,P<0.05;2013-2015年不良反应发生率分别为1.48%,1.21%及0.81%,2015年输血不良反应发生率显著下降,P<0.05。结论建立信息化输血管理系统,促进临床合理用血,提高用血安全。
Objective To evaluate the clinical efficacy of clinical monitoring and monitoring system for blood using electronic monitoring system. Methods Since January 2013, our hospital implemented the electronic monitoring system for monitoring blood before and after the operation. In January 2015, we implemented the blood transfusion monitoring system. Statistical analysis of 2012-2015 amount of surgery, perioperative blood use, 2013-2015 adverse transfusion reactions. Results The operation volume increased continuously during 2012-2015, but the average blood volume decreased during the perioperative period. The average time between the start of blood delivery and the start of blood transfusion in 2013-2015 were 34.38 ± 11.15, 35.35 ± 10.56 and 28.21 ± 5.262 min, respectively ; The interval time in 2015 was significantly shorter than that of the previous two years, P <0.05; the rate of blood transfusion in two hours of 2013-2015 was 41.2%, 43.5% and 72.3% respectively, and the prompt response rate in 2015 was significantly higher than that in the previous two years, P <0.05; the incidence of adverse reactions in 2013-2015 were 1.48%, 1.21% and 0.81%, respectively. The incidence of adverse reactions of transfusion was significantly decreased in 2015-2015 (P <0.05). Conclusion The establishment of information-based blood transfusion management system to promote clinical use of blood, improve blood safety.
