目的:探讨固定矫治器配合改良平面导板矫治青少年安氏Ⅱ类1分类错的临床疗效。方法:选择低角或均角的安氏Ⅱ类1分类错牙合畸形患者12例作为实验组,男6例,女6例,年龄12-17岁,平均14.5岁。采用直丝弓固定矫治,治疗中辅助使用改良上颌平导。选择与实验组相匹配的12例患者作为对照组,仅使用弓丝打开咬合。咬合打开后,制取模型,并进行模型测量分析,分析咬合打开时间及咬合打开量。治疗结束后进行头影测量,评价治疗效果。结果:模型测量分析显示:改良平面导板打开咬合效果显著,覆牙合平均减小2.8 mm,覆牙合平均打开时间为3.75个月,短于对照组,差异有统计学意义(P<0.05)。治疗前后头影测量结果显示:U1-SN由112o减小到101.8o(P<0.05),而U1-L1由109.6o增加到125.3o(P<0.05)。结论:在青少年安氏Ⅱ类1分类错矫治中辅助使用改良平面导板,能快速打开咬合,缩短疗程;同时,能提供较稳定的支抗,达到满意的治疗效果。
Objective: To investigate the clinical effect of fixed appliance combined with modified plane guide in the treatment of Class Ⅱ division 1 malocclusion in adolescents. Methods: Twelve cases of Angle Class Ⅱ Division 1 malocclusion with low or average angle were selected as the experimental group. There were 6 males and 6 females, aged 12-17 years (average 14.5 years). The use of straight wire fixation, the treatment of auxiliary use of the maxillary flat guide. Twelve patients who matched the experimental group were chosen as the control group, only the arch wire was used to open the bite. After the bite opened, a model was taken and model measurement analysis was performed to analyze the occlusion opening time and the occlusion opening amount. Cephalometric measurement after treatment to evaluate the treatment effect. Results: The results of model measurement and analysis showed that the effect of opening and closing bite was improved obviously, the average length of overbite closure was 2.8 mm and the average opening time of overbite closure was 3.75 months, which was shorter than that of control group (P <0.05) . Cephalometric measurements before and after treatment showed that U1-SN decreased from 112o to 101.8o (P <0.05), while U1-L1 increased from 109.6o to 125.3o (P <0.05). Conclusions: The improved plane guide plate is used to assist the correction of Class Ⅱ division 1 malocclusion in adolescents. It can quickly open the bite and shorten the course of treatment. At the same time, it can provide more stable support and achieve satisfactory therapeutic effect.
