目的了解儿童急性髓系白血病(AML)的临床特点,探讨不同化疗方案疗效及预后.方法对86例AML患者随机分成A、B.C三组.分别以不同的化疗方案治疗,对各组诱导治疗期和持续缓解期的完全缓解率及长期无病存活率进行统计分析.结果86例AML患者诱导治疗期和持续缓解期的完全缓解率及长期无病存活率分别为74.42%、67.19%和55.81%;其中,以HD-AraC为主的联合方案(C组)优于其它方案,C组诱导治疗期和持续缓解期的完全缓解率及长期无病存活率均高于A、B两组(P均<0.01);各组都存在不良反应,主要为骨髓抑制、败血症、呼吸道和消化道感染,经积极治疗均及时得以控制.结论以HD-AraC为主的联合方案治疗儿童AML可进一步提高疗效,患儿能够耐受该化疗方案的强度,值得临床应用.
Objective To understand the clinical features of acute myeloid leukemia (AML) in children and explore the curative effect and prognosis of different chemotherapy regimens.Methods A total of 86 AML patients were randomly divided into A and BC groups, treated with different chemotherapy regimens, And the complete remission rate and the long-term disease-free survival rate in the continuous remission period were statistically analyzed.Results The complete remission rate and the long-term disease-free survival rate in induction and treatment of 86 AML patients were 74.42%, 67.19% and 55.81% ; Among them, the combination of HD-AraC-based programs (group C) is superior to other programs, the complete remission rate and long-term disease-free survival rate of induction therapy group and continuous remission phase in group C were higher than those in group A and B All <0.01) .There were adverse reactions in all groups, including myelosuppression, sepsis, respiratory tract and gastrointestinal tract infection.After active treatment, all the patients were controlled timely.Conclusion HD-AraC-based combination regimen can improve the therapeutic effect of AML , Children can tolerate the intensity of the chemotherapy regimen, it is worth clinical application.
