目的探讨磁敏感加权成像对脑星形细胞瘤术前分级诊断的价值。方法分析39例经手术病理证实为脑星形细胞瘤患者的磁敏感加权成像(SWI)的影像表现,对瘤内磁敏感信号(ITSS)级别进行分级并判断肿瘤的高、低级别,与病理结果进行对照。结果 11例低级别组星形细胞瘤中,ITSS评分0级6例,1级5例。28例高级别组星形细胞瘤中,ITSS评分1级2例,2级7例,3级19例。高低级别组星形细胞瘤的ITSS级别有统计学差异(Z=-4.986,P<0.01),ITSS级别与星形细胞瘤的病理级别呈正相关(r=0.958,P<0.01)。结论不同级别脑星形细胞瘤内ITSS级别有明显差异,ITSS级别对星形细胞瘤术前分级有重要价值。
Objective To investigate the value of magnetic resonance weighted imaging in preoperative staging of astrocytoma. Methods Thirty-nine patients with brain astrocytoma confirmed by surgery and pathology were analyzed with MRS. The level of ITSS in the tumor was graded, and the high and low grade of tumors were evaluated. Results were compared. Results Among the 11 low-grade astrocytoma cases, ITSS score was 0 grade 6 cases and grade 1 5 cases. Of the 28 high-grade astrocytomas, ITSS score was 1 grade in 2 cases, grade 2 in 7 cases and grade 3 in 19 cases. There was a significant difference in ITSS grade between high and low grade astrocytoma (Z = -4.986, P <0.01). ITSS grade was positively correlated with pathological grade of astrocytoma (r = 0.958, P <0.01). Conclusion The levels of ITSS in different grades of astrocytoma are obviously different. ITSS grade has important value in the preoperative classification of astrocytoma.
