为提早出院的易感患者提供出院后的家庭随访,这是延续护理的起源,是为了使健康服务更加有效、成本更加低廉。现在的延续护理提供全方位的健康服务给所有出院患者,其中包括家庭访视、电话随访、建立患者俱乐部和延续护理中心、在网络上开展健康教育等等。延续护理正朝着关注心理和精神问题的方向发展,还要对相关的法律法规予以完善。
Providing post-discharge home follow-up for susceptible patients discharged early is the source of extended care in order to make health services more cost effective and cheaper. Current Continuing Nursing provides a full range of health services to all discharged patients, including family visits, telephone follow-up, establishment of patient clubs and extended care centers, health education on the web, and more. Continued nursing care is moving in the direction of psychological and spiritual concerns, but also to improve the relevant laws and regulations.
