目的通过检测脑脊液及血清碱性髓鞘蛋白(MBP)抗体水平,及观察外周血单个核细胞的白细胞介素(IL)-2、转化生长因子(TGF)-β的mRNA表达,探讨多发性硬化(MS)免疫机理以及其临床应用的价值。方法病人组为临床确诊的MS42例,其他神经系统疾病(OND)组25例,对照组为健康成人或腰麻手术病人20名。对上述对象皆收集其血清标本。对MS组中的31例、OND组中的25例及对照组中的14名采取脑脊液。采用ELISA方法检测MBP抗体的水平。分别以对照组脑脊液和血清MBP抗体水平的x-+2s为阳性标准,计算其余两组的阳性与阴性数目。采用逆转录-PCR(RT-PCR)方法检测24例MS病人及15名对照组外周血单个核细胞中IL-2、TGF-β的mRNA表达。其中MS组病人活动期15例,稳定期9例。结果MS组脑脊液和血清的MBP抗体水平均比对照组增高,统计学处理示差异有显著性意义(P<0.01)。MS组的脑脊液和血清MBP抗体水平较OND组为高,二者经统计学检验差异有显著性意义(P<0.01)。在MS组中,脑脊液标本MBP抗体的阳性率虽高于血清(分别为80.7%和64.3%),但差异无统计学意义。在OND组,脑脊液和?
Objective To detect the level of anti-MBP antibody in cerebrospinal fluid (CSF) and serum and the mRNA expression of interleukin (IL) -2 and transforming growth factor (TGF) -β in peripheral blood mononuclear cells (MS) immune mechanism and its clinical value. Methods The patient group was clinically diagnosed MS42 cases, other neurological diseases (OND) group 25 cases, the control group of healthy adults or spinal anesthesia surgery patients 20. Serum samples of all the above subjects were collected. Cerebrospinal fluid was taken in 31 of the MS group, 25 of the OND group and 14 of the control group. The level of MBP antibody was detected by ELISA. Respectively, the control group of cerebrospinal fluid and serum MBP antibody levels of x- + 2s as a positive standard, calculate the remaining two groups of positive and negative number. The mRNA expressions of IL-2 and TGF-β in peripheral blood mononuclear cells of 24 patients with MS and 15 controls were detected by reverse transcription-polymerase chain reaction (RT-PCR). Among them, MS patients were active in 15 cases and stable in 9 cases. Results The levels of MBP antibodies in CSF and serum in MS group were significantly higher than those in control group (P <0.01). The level of CSF and serum MBP in MS group was higher than that in OND group, and the difference was statistically significant (P <0.01). In the MS group, the positive rate of MBP antibody in cerebrospinal fluid was higher than that in serum (80.7% and 64.3% respectively), but the difference was not statistically significant.
